The purpose of the essay is to look into the fundamental principles of person-centred care approach and how it is applied by nurses in care planning. The author will use the nursing process framework to create a care plan for a chosen scenario. Significantly within the essay, the writer will also discuss the befits of using structured and goal-orientated nursing process for patients and the multidisciplinary team. The author has chosen Vipul Patel scenario (for further information about the scenario please refer to appendix 1) because cardiology has always been a field that she had interest in. However, the main reason the author has chosen this scenario is because her grandmother has died during her second heart attack only couple years ago. The author strongly believes that providing person-centred care improves the communication between both sides, which automatically minimise errors and omissions in care planning. After reviewing Vipul Patel scenario, the author has decided that the essay will address three main issues which are chest pain, obesity and anxiety. However, the author will write a care plan to reduce Vipul’s Patel chest pain as it is the main priority. The people within this assignment are factitious, therefore the author will refer to the Code of Professional Conduct in order to maintain confidentiality. (NMC, 2015)Main body of the essayIt is fundamentally important to understand the philosophy of person centred approach, in order to provide high quality of care. Therefore, the author will discuss what person centred care is and its importance in healthcare setting.A person centred care is simply individualised approach to the person, ensuring that regardless of the circumstances, patient’s needs, preferences and wants are respected and obtained. It also includes providing all the information, the patient needs to make decisions regarding their own health and supporting them to participate in decision making. The term person-centred care roots to humanistic psychotherapy. The research shows that the term “person-centred care” was known as “client-centred therapy “that originates from American psychologist Carl’s Roger. In late 70’s Roger has produced a book called ‘Carl Rogers on Personal Power (1977), where he discussed the applications of the person-centred approach to all human relationship and its effectiveness to psychotherapy. He deliberated on constructive fulfilment of inherent possibilities and highlighted that the distribution of power and influence in all human relationships will encourage individuals to make decisions related to their lives and health. Furthermore, the studies on person centred approach in healthcare setting carried out by Professor Brendon McCormack and his colleagues have gained world-wide recognition. McCormack (2008) stated that person centred care is an approach to practice established through the formation and fostering of therapeutic relationships between all care givers, care receivers and all other significant to them in their lives. It is underpinned by values of respect for persons, individual right to self-determination, mutual respect and understanding. It is established by cultures of empowerment that foster continuous approaches to practice development. Their work does not only highlight the importance patient-nurse relationship but also looks at the care environment and effective communication and shared values within the multidisciplinary team. These and other concepts resulted in creating person-centred framework for nursing. Moreover, person-centred training courses were developed based on the framework.
